Applications to Enrol at Lesmurdie Primary School can be made at any time during the year, by completing an Application for Enrolment, Enrolment Form and Student Healthcare Summary.

Please also complete additional information for enrolling Kindy students.

As per the Department of Education’s Enrolment Policy parents will need to provide the following documents at the time of enrolment:

Evidence of the child’s legal name ie. birth certificate, extract of birth certificate or passport

• Immunisation records from myGov
• Immigration records, if applicable
• Proof of address
• Medical information

Students will be placed in the year of study according to their date of birth. Parents of children with special needs are asked to contact the school to discuss these needs when the Application is made.
